**Building, plumbing, tree, & street permit applications must be printed on legal sized paper as well as the Nassau County Dept. of Assessment form.**
You may also pick up an application at Village Hall. We are located at 55 Manhasset Avenue, Manhasset, NY 11030.
Building Permit Application:
An original application on legal sized paper must be submitted.
Note: All Building Permits must be accompanied by a Short Environmental Assessment Form.

All permit applications need to be accompanied by:
- General Liability Insurance certificate (accord form
- Worker’s Compensation Insurance Certificate (U-26.3 state form) (issued by the NYS Insurance Fund
- Disability Insurance Certificate (DB-120.1 state form) (issued by the NYS Workman’s Compensation Board
- Copy of Contractors Nassau County or Town License
**ALL CERTIFICATES MUST LIST THE Village of Plandome Manor, 55 Manhasset Avenue, Manhasset, NY 11030 as the Certificate holder & Additional insured.
